To make areas where the characters can't walk to, you need to make collisions which are usually invisible in the game. In other words use the Editor's type search to look for Collision Helper and place it. You should be able to find help for this from our Wiki, I also recommend checking out the videos linked there.
And to your third question, unfortunately there seems to be some kind of bug when placing lava. If you'd like to use lava you can get around this by copying the lava from another level, for example from fightchallenge_03. Sorry for the inconvenience!
